Application Process
Required Documentation
Each family must submit the following for their file. To access the forms online, click here.
- One application for The Miriam Project.
- One original Birth Certificate and one copy per parent
- One copies of the Marriage Certificate
- One copies of a Divorce Decree (if applicable)
- One Miriam Project Financial form
- One Miriam Project Medical form per parent
- Photos of the family — one of each individual and one of the family
- Two copies of the home study report (supplied by The Miriam Project)
- Child Protection Service reference checks for each adult in the home from each county in the past 10 years
- Four non-family references with one being from your minister or pastor of faith
- Two copies of the letter to the birth parents
- Statement of medical insurance coverage for adopted child
- Statement of explanation about any children no longer living in the home
- Release of Confidential Information as needed
- Criminal History check — request will be made by The Miriam Project
- Checklist for Special Needs Child
- Checklist for Domestic Transracial/Transcultural Child
- Additional information may be requested
