Application Documents
The following documents need to be completed to begin the adoption process with The Miriam Project. You will need Acrobat Reader view and print the documents.

Anderson, IN 46012Remember to sign the Statement of Faith at the end of the application. Please submit the application fee of $175.00 (payable to: Miriam Project) along with your application.
- Medical History
- One for the husband and one for the wife. Take it to your physician for completion. Upon completion, return them to the above address with the application form. They must be completed within 6 months of the initial application.
- State-wide Criminal History
- Please review and submit the required request form to the address provided.
- Financial Profile
- Complete and return with the application form.
- References
- Four non-family references with one being from your minister/pastor from your faith community. Complete the top part and forward to the Referee with a stamped envelope addressed to the Miriam Project.
- Data Collection
- Complete and return with the application form.
